All enrollments are currently on a waitlist. The enrollment link will be sent via email once space is available.

Once confirmed, you’ll be able to schedule your first visit right away

Your card will not be charged until enrollment is confirmed. Billing occurs with activation and on the first of every month.

    • We are located in Golden, CO and currently serve Colorado residents and additional select locations - see below. Care for additional states is pending, based on need.

      • Washington state - both Dr. Matthews and Dr. Bross

      • Arizona, Utah, Montana, Wyoming - Dr. Bross

    • Membership is for children 14+ and adults of all ages.

    • We currently cannot enroll patients with any form of Medicare insurance due to previous contracts. We are actively attempting to remedy this.

      • Please send an inquiry to be added to a Medicare waitlist and to stay up-to-date on this process.

    • DPC is not a replacement for emergency services or hospital care.

    Membership fee withautomated billing

    • Young adults ages 14-25: $100/month or $1100/year

    • Adults ages 26-64: $150/month or $1650/year

    • Adults ages 65+: $200/month or $2200/year

    • Family Care: Although there is no “bundled family rate”, we already discount adolescents and young adults (see rates above) and your family members are referred members! Take advantage of these savings while keeping your family care all in one place - we practice family medicine for a reason!
