• Membership is for children 14+ and adults of all ages.

• We currently cannot enroll patients with any form of Medicare insurance due to previous contracts. We are actively attempting to remedy this.

  • Please send an inquiry to be added to a Medicare waitlist and to stay up-to-date on this process.

• DPC is not a replacement for emergency services or hospital care.

• Young adults ages 14-25: $100/month or $1100/year

• Adults ages 26-64: $150/month or $1650/year

• Adults ages 65+: $200/month or $2200/year

3. Once confirmed, you’ll be able to schedule your first visit right away.
